CoC Chronic Homelessness Workgroup

The Chronic Homelessness Workgroup meets to combine efforts and strategize to prevent and end Chronic homelessness in the San Antonio / Bexar County region. The goals of the workgroup are:
1. Document progress towards achieving USICH Benchmarks: Continue compiling documentation of progress towards achieving USICH Benchmarks of Addressing and Ending Chronic Homelessness by engaging with key stakeholders to assess and determine the stock of available services dedicated for persons experiencing chronic homelessness, and to increase the collaboration between homeless service providers, to include Outreach staff, in order to demonstrate completion of Benchmark #1 in the next 12 months. At least 75% of chronic workgroup participants who attend 75% or more of monthly meetings shall participate in recording documentation towards at least (1) Benchmark over the next (12) months.
2. Launch Move-On Pilot with SAHA: Engage with SAHA to launch the “Move On” pilot program, as well as establish a Move On Review Board panel of PSH Directors to review and refer PSH participants for a dedicated HCV under the Move On program. Utilize and house (40) formerly chronically homeless households through the Move On program over the next 12 months. Collaborate with direct service providers, Outreach staff, and Navigation staff to ensure the process from the referral to the Move On program to being successfully housed with a dedicated HCV voucher under the Move On program occurs in less than 90 days.
3. Create a data monitoring process: Establish a data monitoring plan for the Chronic Homeless population reported through HMIS to ensure that accurate data is being captured through the HUD Universal assessment and ensure that the number of chronically homeless households being reported through HMIS is accurately represented. Ensure that the data monitoring plan includes a monthly monitoring schedule and a dedicated process for correcting inaccurate data on a monthly basis.
